Required elements of a takedown notice (17 U.S.C. §512(c)(3))
- A signature (physical or electronic) of a person authorized to act on behalf of the copyright owner
- Identification of the copyrighted work claimed to have been infringed
- Identification of the material that is claimed to be infringing, with information sufficient to locate it
- Contact information for the complaining party (address, phone, email)
- A statement, made under penalty of perjury, that the complaining party has a good-faith belief that the use is not authorized
- A statement that the information in the notice is accurate
Counter-notice procedure
If you believe content was removed in error, you may submit a counter-notice including the elements above plus a statement that you consent to the jurisdiction of the federal district court in DC.

For non-copyright takedown requests (defamation, privacy), use the public takedown form.
