Required elements of a takedown notice (17 U.S.C. §512(c)(3))

  1. A signature (physical or electronic) of a person authorized to act on behalf of the copyright owner
  2. Identification of the copyrighted work claimed to have been infringed
  3. Identification of the material that is claimed to be infringing, with information sufficient to locate it
  4. Contact information for the complaining party (address, phone, email)
  5. A statement, made under penalty of perjury, that the complaining party has a good-faith belief that the use is not authorized
  6. A statement that the information in the notice is accurate

Counter-notice procedure

If you believe content was removed in error, you may submit a counter-notice including the elements above plus a statement that you consent to the jurisdiction of the federal district court in DC.
